Intake Procedure
(1) Call or go online and ask to refer self or someone else for services (2) Counselor of the day will take down information and entered into the case management system (3) Applicant will be assigned a counselor and will be contacted by phone or through mailed letter (4) Initial interview will be scheduled to discuss program details, review medical and psychiatric records, discuss goals for the future and available services (5) Any additional documentation needed will be discussed

1) Call or go online and ask to refer self or someone else for services (2) Staff will take down information and entered into the case management system (3) Staff will be assigned to visit applicant at home to discuss eligibility and available services and complete application (4) Staff will obtain all needed or releases of information, and copies of Photo ID (5) A service plan will be developed and applicant will be assisted in locating in-home service providers
