Required Documents
Photo ID: Passport or Legal Permanent Resident card (Green Card); Proof of Income; Proof of address.
Eligibility
Must reside in DuPage County; Must be a U.S. citizen or a Legal Permanent Resident; Does not qualify for Medicaid or Medicare; Able to enroll in a Marketplace Health Insurance Plan.

Intake Procedure
(1) Apply and enroll in health insurance from www.healthcare.gov, (2) Choose a Silver Level plan, (3) Use entire Advance Premium Tax Credit (APTC), (4) Select a plan from any insurance company available for the current year, (5) Apply for Silver Access.
