Therapeutic activities and services for older adults who are no longer able home alone during the daytime.
Hours
Monday to Friday, 7:30 am - 5:30 pm
Eligibility
Individuals 50 years and older (exceptions made for specific situations); Must be bladder and bowel continent and/or wear protective hygiene undergarments; Must be able to move with or without an assisted device; Must be able to take part in the activities in a social model program.
Fees
Self-pay, Private Insurance, No Fees (for those eligible for the Community Care Program or are registered with the VA).
Intake Procedure
(1) Call 630-968-1060 to schedule a tour of the center and an interview, (2) Complete membership application forms including physician assessment and application fee.
