(1) Completed application, (2) Two forms of identification, except in case of homeless individual: (a) One must include his/her residence address, (b) Driver's license, social security card, public aid identification card, utility bill, employee or student identification card, lease or contract for a residence, credit card, or a civic, union or professional association membership card, (c) Affidavit of Resident if unable to provide ID with current residence.

(1) Resident of DuPage County, (2) United States citizen, (3) At least 18 years old on or before the next election, (4) Must not be convicted and in jail, (5) Cannot claim the right to vote anywhere else
