Office: Monday to Friday, 9:00 am - 5:00 pm; Intake Line: 7 days a week, 24 hours a day

Intake Procedure
(1) Any parent or agency calling on behalf of a parent can call our intake line to begin the intake process: 773-653-2200 (2) Submit online Parental Consent and Child Intake forms. (3) After a screening, intake office matches host families with families in need, if it is a situation we are able to help with.
