Hours:
Monday to Friday, 8:30 am - 4:30 pm
Intake Process:
(1) Call 1-888-235-5826 and leave your name, mailing address and zip code to receive an application OR complete an online application from the website
(2) If application receives an initial approval, applicant will be put on a waiting list in the order the application was received.
(3) A DDS program coordinator will reach out to the applicant and refer to a volunteer
(4) Program volunteer will assess the applicant and schedule an appointment.
(5) Final acceptance into the program will be complete after the first appointment with the dentist.
