Eligibility
Served in active military, naval or air service; Separated under any condition other than dishonorable; Current and former members of Reserves or National Guard who were called to active duty and completed the full period for which they were ordered may also be eligible; Veterans who enlisted after September 7, 1980 or entered active duty after October 16, 1981 must have served 24 continuous months or the full period for which they were called to active duty

(1) Visit www.hines.va.gov/patients/eligibility.asp to complete an "Application for Health Care Benefits". (2) Submit the form online, mail it to Hines VA Hospital, Eligibility, 5000 South 5th Avenue, Hines, IL 60141, or bring the form to the hospital. (3) Include a copy of both sides of your current insurance card and a copy of your DD214 "Armed Forces Report of Transfer or Discharge"
