Provides referrals to providers of services for service members, veterans and their families for employment, education, healthcare, benefits, emergency assistance, legal support, housing, women veteran support, family and survivor support, veteran recreation and volunteering.

Intake Procedure
(1) Call the Care Coordination Center to connect with a specialist, email, or fill out Assistance Request Form on website (2) Care Coordination Specialist will assess the request for assistance to determine needs, and then make referrals to providers within the Veteran Support Community

Illinois Warrior Assistance Program (IWAP) is a Warm Helpline provides confidential assistance for Illinois veterans to help them meet the challenges of their everyday life. IWAP is also available to family members including the spouse and children of eligible veterans.
