(4) Full level of care assessment will be conducted and a recommendation will be made
(5) Documentation required:
a. State or Federal ID

d. List of current prescriptions
e. List of important phone numbers
f. Medical Power of Attorney or Guardianship Paperwork if needed
g. Legal documents proving legal guardianship for a minor
(6) Patient will be given a unique ID number when admitted
