Monday to Friday, 8:00 am - 4:30 pm

Required Documents

(1) List of medications you are currently taking, including name(s) and dosage(s) (2) Proof of identification (a valid photo ID, birth certificate or U.S. Passport) (3) Proof of insurance (if applicable) or Medicaid or Managed Care card (4) Proof of income (Examples: child support, pensions, social security card, statement of wages, unemployment (5) If no income, proof of income (90 days) for the person(s) helping you financially and a letter of support written by them (6) If self-employed: Recent Federal Tax Returns and three months recent bank statements showing deposits of income (7) If paid in cash: three months of bank statements showing the deposits of income. If you file taxes, you may also use them with the employer letter (8) Proof of address (provide one of the following: a utility bill issued in the last 30 days (electric, gas, water), current lease or proof of mortgage, OR if you live with someone else and have none of the documents listed above to prove your address: a. Provide a letter from the person you live with indicating you reside with him/her, and b. Have the person writing the letter provide one of the above proofs of address documents in his or her last name (9) Optional: Health History in English or Spanish (PDF document found online)
