2 forms of identification (needed during each visit): a. Photo ID, b. Verification of address (current utility bill with address, rent agreement); A program ID card/punch card (issued after intake is completed)

Verification of address during intake (current utility bill with address, rent agreement) (3) A program ID card/punch card is issued after intake is completed (4) 1 visit allowed per month
