Intake Process:
(1) Contact Program Manager to complete a phone screening at which time an appointment may be scheduled
(2) Walk-ins are not accepted
(3) Refer to service details for required documentation

Eligibility:
(1) One (1) day of active duty service
(2) Greater than dishonorable discharge
(3) Must have a permanent housing plan or have a mental health/substance use diagnosis
(4) No history of convictions for sex-related crimes and no history of violence or aggressive behavior in the past five years

Photo ID; DD214; Social Security card; Income verification for previous 6 months; Bank statements for previous 6 months.
Eligibility
One (1) day of active duty service; Greater than dishonorable discharge; Must have a permanent housing plan in place or have a mental health/substance use diagnosis; No history of convictions for sex-related crimes and no history of violence or aggressive behavior in the past five years.
