Completed application; Two forms of identification, except in case of homeless individual: (a) One must include his/her residence address, (b) Driver's license, social security card, public aid identification card, utility bill, employee or student identification card, lease or contract for a residence, credit card, or a civic, union or professional association membership card, (c) Affidavit of Resident if unable to provide ID with current residence.

Resident of DuPage County; United States citizen; At least 18 years old on or before the next election; Must not be convicted and in jail; Cannot claim the right to vote anywhere else
