For commercial airline tickets: (1) Completed medical clearance form from doctor, (2) Income verification: Social security numbers should be blacked out from documents
Eligibility
(1) Patients must be ambulatory (able to walk independently, enter and exit the plane with little or no assistance) and require no medical care (from doctor, nurse, etc.) en route to destination, (2) If patient requires oxygen, unless provided by the airlines, patient must have a portable oxygen concentrators (POC) approved by the FAA or the airlines, a. Patient is responsible for making these arrangements and paying for travel fees associated with the oxygen, (3) Meet income guidelines
