Description
Illinois Supportive Living Program (SLP) - State program offering an alternative to nursing home care for older, low-income residents and those with physical disabilities covered by Medicaid. Offers web listings of all operational SLP providers in Illinois, who offer elderly (65 and older) or persons with physical disabilities (22 - 64 years old) personal care and other services.

Eligibility
Must be either a U.S. citizen or legal resident; Resident of Illinois; 22 years of age and older living with physical disabilities (as determined by Social Security Administration), or age 65 years and older; Screened by IHFS or an authorized screening agency and identified as someone in need of nursing facility level care; Cannot be participating in any other home and community-based services waiver; Income is greater than or equal to current maximum allowable amount of the Supplemental Security Income (SSI) and must contribute a fixed amount from that income monthly towards living, meal and service expenses.
