Eligibility
Blind persons whose visual acuity is 20/200 or less with correcting lenses, or with a visual field of less than 20 degrees; Other physically disabled persons including: a. Persons whose visual disability is certified as preventing reading, b. Persons certified as unable to read or unable to use printed material because of physical limitations, c. Persons certified as having a reading disability severe enough to prevent reading in a conventional manner

(1) Download application from website, complete and send by mail (2) Application must be certified by health care practitioner including home health care providers, nurses, social workers, librarians - listed on application as eligible for the program
