Description
Hospice care can happen at home, in hospitals, or in skilled nursing homes for people with long-term illnesses like heart disease, COPD, cancer, dementia, stroke, AIDS, and others. The services include managing medicines, easing symptoms, helping with medical equipment, and giving emotional and spiritual support to patients.

Eligibility
Diagnosed with a chronic illness with 6 month prognosis; Must meet hospice requirements as per Medicare guidelines

(1) Call during posted hours to make a referral or inquire about receiving services (2) The office will check insurance, eligibility, and benefits (3) The hospice team will schedule an initial visit with the patient and their family.
